Employee's medical claim submission - Clinical

In the medical profession, a general practitioner (GP) is a medical doctor who treats acute and chronic illnesses and provides preventive care and health education to patients.

If you provide a GP claims to you employee, this claims pays for the clinical outpatient consultation, medical fees, outpatient diagnostic x-ray and laboratory tests when the Insured Member visits a General Practitioner under the FWD panel of clinics or polyclinics.
 

(a) My employee's medical coverage cover outpatient treatments

Great! This means the medical insurance you bought cover outpatient treatments that are done without staying in the hospital. Advise your employee to carry the medical card provided to any panel clinic.

(b) My employee's medical coverage did not cover outpatient treatments

If your employee's medical plan did not cover for outpatient, your employee still can go to any clinic and claims. 
 
Follow these simple steps to expedite the filing of a medical claim.
 

Step 1 - Contact Swingvy team to get the right claim form.

To file a claim you must first obtain the correct claim form. It could be very confusing which form need to be completed. Contact Swingvy Insurance team for assistance by email to support@swingvy.com
 
Swingvy tip: 
Upon receiving the form you need to fill-up the form policy number, name of claimant, name of the hospital, date of admission or accident, diagnosis/accident as soon as possible but no later than 30 days.

 
Step 2: Collect important documents from your employee

Clinical Claim - General Practitioner

  • Original receipt 
  • Details of charges (for bill above RM75) 

Step 3 - Submit your claim to Swingvy
